Endodontist Insurance Information

At Endodontic and Implantology Associates we make every effort to provide you with the finest surgical care and the most convenient financial options. To accomplish this goal, we work hand in hand with you to maximize your insurance reimbursement for covered procedures.

If you have dental insurance, as a courtesy, our administrative staff will try to acquire as much benefit information as possible from your carrier. Please note that any benefit information acquired is only an estimate and any outstanding claims by other dental care providers cannot be taken into accurate account.

We are providers for:

  • Delta Dental
  • Delta PMI
  • Pacific Union Dental
  • Liberty Dental

If you have another insurance carrier other than the above noted, please contact us for more detailed information about coverage.

For Delta PMI, PUD, and Liberty Dental enrollees please make sure to acquire and bring with you the required authorization paperwork from your referring dentist to your first appointment. This will be necessary in order to initiate consultation or any treatment.
